If any of the above is not specified, the defaults are upright, no head counter, all weapons holstered and uncocked. The defaults will be applied individually. That is, if you specify "down" but nothing else, you will be down with no head counter and all weapons holstered and uncocked.
All characters setup in the bar building on board HH. Setting up on the bar (or any obstacle, for that matter) is legal.

This is your last warning, BE default is VITAL, meaning a kill (and -6 VPs). So be SURE to specify the desired BE location when you shoot.
